It looks like improved transition care for frail seniors in Espanola is on the way. An announcement is planned for this Thursday at the Espanola Seniors Centre. The Local Health Integration Network for the Northeast is making the announcement. The news centres on improving the transition for frail seniors from a hospital setting to a non hospital environment. Among the speakers are Ray Hunt, who is CEO of the Espanola Regional Hospital and Health Centre.
